Is There a Clinical Difference between Salt Water and Fresh Water Drowning?

چکیده
Introduction: Salt water near-drowning events are not uncommon in coastal regions. The difference in tonicity between salt and fresh water in near-drowning events leads many to believe there should be differences in the approach to managing the patient. We present a case of near-drowning in salt water and a thorough review of the literature in managing these patients. Case Presentation: A sixteen year old male presented to our level 1 trauma center approximately 30 minutes after a near-drowning in salt water. The patient began to develop hypoxemia associated with profound pulmonary edema that was not responsive to elevated inspired oxygen levels and high levels of positive end-expiratory pressure (PEEP). Conclusion: Despite the alterations in the alveolar interface that are dependent on the tonicity of the fl uid, the ultimate treatment of drowning injuries remains consistent regardless of the tonicity of the water. Aggressive therapy, tailored to the individual patient, can improve pulmonary shunting. Patients with hypoxemia requiring intubation should be ventilated with (PEEP) as animal studies have shown rapid improvements in PaO2 with PEEP values in the range of 5-15. Secondary drowning in children.
Secondary drowning (and near-drowning) is one of the post-immersion respiratory syndromes. It is defined as deterioration of pulmonary function that follows deficient gas exchange due to loss or inactivation of surfactant. A review of 94 consecutive cases of near-drowning in childhood showed that this syndrome occurred in five (5%) cases. Its onset was usually rapid and characterised by a laten...

“Drowning” is defined as “suffocation by submersion, especially in water”66; it continues to be the third most common cause of accidental death in the general population and, for children in many countries, the second most common cause after road accidents.75 In the USA, 40% of drowning victims are less than 4 yr old.74 In Britain, the Office of Population Census reports that child deaths from ...

Background: Anthropogenic activities release high concentrations of heavy metals into the aquatic ecosystems, which can be absorbed by the aquatic organisms. In this study, the accu-mulation of cadmium (Cd) and arsenic (As) was compared in liver, gill and muscle tissues of toothed carp (Aphanius sophiae) in fresh and salt water. Methods: A total of 175 fish samples were collected from the Sh...

Drowning is potentially fatal. Physiologic responses to aspiration of water differ according to whether the water is salt or fresh; whether these responses are significant depends on the volume of fluid aspirated. Resuscitation efforts should be started at the scene of the accident. Oxygen therapy is essential. Many patients require continuous positive airway pressure or positive end-expiratory...
متن کاملDrowning Syndromes: the Mechanism
The drowning syndromes should be viewed as a continuum between the aspiration of a relatively small amount of water, causing symptoms and respiratory-based signs, through near-drowning, in which there is loss of consciousness but with survival, to the fatal cases of drowning. The latter rarely involve the gross haemodynamic and biochemical changes seen in some animal experiments. The behaviour ...
